螺旋CT肝动脉期原发性巨块型肝癌增强血管形态分析

目的探讨螺旋CT肝动脉期扫描原发性巨块型肝癌肿瘤内增强血管形态及与其生物学特性关系.材料和方法回顾性分析了43例原发性巨块型肝癌患者的螺旋CT肝动脉期扫描资料.结果螺旋CT肝动脉期原发性巨块型肝癌肿瘤内增强血管的形态分为6种类型(1)肿瘤中心单枝型5例(11.63%);(2)肿瘤中心不规则多枝型10例(23.25%);(3)肿瘤内弥散不规则条状为主型13例(30.23%);(4)肿瘤内弥散结节为主型3例(6.97%);(5)肿瘤内周围型9例(20.93%);(6)均一增强型3例(6.97%).第4型易侵犯门静脉.第5型易发生中心坏死区,边界多较清,形态多较规则.第6型血供最丰富.结论螺旋CT肝动脉扫描可反映原发性巨块型肝癌肿瘤内增强血管的形态类型,对了解其生物学特性有一定的意义.

Analysis of Intertumor Enhanced Vessels of Primary Huge Liver Cancer with Hepatic Helical Artery-phase CT Scanning

Purpose: To discuss the enhanced vessels types of primary huge liver cancer with hepatic helical artery phase CT scanning and relationship to biologic features of primary huge liver cancer. Materials and methods: The hepatic helical artery phase CT scanning date were reviewed retrospectively in 43 patients with primary huge liver cancer. Results: According to the CT appearance of hepatic helical artery phase, the enhanced vessel types of primary huge liver cancer were divided six types: (1) single branch vessel in center region of tumor in 5 (11 635%). (2) poly branch vessel in center region of tumor in 10 (23.25%). (3) diffuse, irregular enlarge branch vessel in tumor in 13 (30.23%). (4) diffuse, enlarge, nodose, small spot, branch supply vessel in tumor in 3 (6.97%). (5)irregular, enlarge, branch and small spot supply vessel in the periphery of tumor in 9 (20.93%). (6) marked homogeneous enhancement of tumor in 3 (6.97%). The fourth types of primary huge liver cancer were more involvement of portal vessels. The fifth types of primary huge liver cancer had more center necrosis region, with regular shape and well margin. The sixth types of primary huge liver cancer were more blood supply. Conclusion: Hepatic helical artery phase CT scanning could demonstrate the enhanced vessels types of primary huge liver cancer. It is of great significance for learning the biologic features of primary huge liver cancer.
