| 摘 要： | 目的评价改良胆管空肠襻式吻合治疗胆道梗阻的临床疗效。方法将92例胆道梗阻病例根据病种随机分为两组，治疗组手术方式采用改良胆管空肠襻式吻合，对照组手术方式采用胆管空肠Roux—en—Y吻合，观察术后反流性胆管炎、肛门恢复排气时间、胆漏、总胆红素下降指标及发生胆肠吻合口狭窄情况。结果术后并发症（反流性胆管炎、胆漏、胆肠吻合口狭窄）发生率对照组为21．7％，治疗组为8．7％，差异有统计学意义（P〈0．05）；术后第1天肛门恢复排气对照组为2．2％，治疗组为7．4％，差异有统计学意义（P〈0．05）；术后2周总胆红素下降50％以上对照组为82．6％，治疗组为91．3％，差异有统计学意义（P〈0．05）。结论改良胆管空肠襻式吻合治疗胆道梗阻后术后并发症减少，胃肠功能恢复快，手术简单、省时，值得临床应用。
