封闭负压引流技术联合抗生素骨水泥应用于下肢大面积撕脱伤的疗效

摘    要:

目的  探讨抗生素骨水泥运用于下肢大面积皮肤撕脱伤创面的疗效。方法  筛选该院2013年1月- 2015年5月60例下肢大面积皮肤撕脱伤患者,分为研究组30例和对照组30例,对照组仅用封闭负压引流(VSD),研究采用VSD联合抗生素骨水泥,3~5d创面新鲜后将自体皮回植,观察记录两组术后皮片成活率、创面感染率和平均住院时间。结果  研究组对照组相比,术后皮片成活率分别为(96.67±3.24)%和(73.33±4.86)%,差异有统计学意义(P =0.000);创面感染率分别为(3.33±1.35)%和(30.00±1.56)%,差异有统计学意义(P =0.000);平均住院时间分别为(10.44±2.53)和(20.81±7.42)d,差异有统计学意义(P =0.000)。结论  抗生素骨水泥运用于下肢大面积皮肤撕脱伤能大大降低创面感染率,有效提高创面的植皮成活率,缩短患者住院时间,节约治疗费用,简便安全,值得推广。

Abstract:

Objective To investigate the clinical effect of antibiotic bone cement in the treatment of large area skin avulsion injury of lower limbs. Methods From January 2013 to May 2015, 60 cases of lower limb large area skin avulsion injury patients were randomly collected into study group (30 cases) and control group (30 cases). The control group was treated with vacuum sealing drainage (VSD), and the research was treated with  VSD combined with antibiotic bone cement, and autologous skin were grafted after 3-5 days. Postoperative skin graft survival rate, wound infection rate, average hospitalization time in two groups were observed and recorded. Results Compared with the control group, the survival rate of skin graft, wound infection rate and average length of hospital stay were shorter in the study group. Conclusions Antibiotic bone cement used in lower limb large area skin avulsion injury can greatly reduce the incidence of wound infection, improve the survival rate of skin grafting wounds effectively, shorten hospitalization time and save the treatment cost, safe and convenient, worthy of promotion.
