髋臼骨折的CT与X线平片诊断比较

目的:比较CT与X线平片对髋臼诊断的价值。材料和方法:18例髋臼骨折病人,全部接受CT扫描确诊,其中,14例是先经X线平片诊断为骨折的,4例X线检查未发现骨折线。分析比较CT与X线平片的表现。结果:经CT扫描发现的5例后壁骨折,X线漏诊2例;1例股骨头前缘皮质凹陷骨折、7例髋臼腔内游离碎片和10例髋关节周围软组织肿胀,X线均未能发现。结论:CT可准确显示骨折线的数目和走行方向,显示骨折类型和关节面损伤程度,以及关节腔内有无骨折碎片等情况,为临床治疗提供比X线平片更可靠的信息。

Comparison between CT and X-Ray Plain Film in the Diagnosis of Acetabular Fracture

Purpose:To compare the use of computed tomography(CT)and X - ray plain film in the dianosis of acetabular fracture. Materials and Methods : Eighteen patients with acetabular fracture initially underwent the examination of X- ray plain film, then again underwent additional CT scans. The diagnostic sensitivity and the manifestation between both methods were analyzed and compared. Results: Acetabular fracture revealed on X - ray plain film were only 14(77% )in 18 patients, because of the fracture line in other 4 patients were could not found so that them were leak -diagnosed. However,CT scans most accurately demonstrated acetabular fracture in all patients. Some acetabular fractures or its manifestalions those were not found on X - ray plain film, such as 2 cases of acetabular posterior wall fracture (5 cases found on CT) , 1 case of depressed fracture in anterior edge cortex of femur head,7 cases of free chip in cotyloid cavity, and 10 cases of soft tissue swelling in periphery of hip joint,etc were clearly revealed on CT images. Conclusion: CT can accurately reveal the numbers of fracture line and its walking direction, type of fracture, injury degree of joint surface, and whether to have fracture chip in joint cavity. therefore, CT can provide reliable informations for clinical therapy of fracture and is more accurate than X - ray plain film.
