2型糖尿病并发无痛性急性心肌梗死的临床研究(附30例分析)

目的探讨2型糖尿病(DM)并发无痛性急性心肌梗死(AMI)的临床特点和诊治对策。方法分析30例患者的临床特点,纠正休克后对患者应用鲁南欣康治疗,观察其疗效。结果2型DM并发无痛性急性心肌梗死的主要临床表现为胸闷、心悸、呼吸困难、倦怠乏力、高血压、恶心、呕吐、心律失常、心力衰竭和心源性休克;6例患者低血糖后发生AMI;合并心源性休克患者的疗效与无心源性休克者比较有统计学意义(P<0.01)。结论低血糖是重要的可预防的诱发因素之一。积极控制DM,预防低血糖,及时确诊无痛性AMI,及早给予鲁南欣康和综合治疗,可明显改善患者的预后。

CLINICAL STUDY ON PAINLESS ACUTE MYOCARDIAL INPARCTION IN 30 CASES OF TYPE 2 DIABETES

Objective To explore the clinical features and the treatments of painless acute myocardial infarction(AMI) in patients with type 2 diabetes.Methods 30 cases of painless AMI were treated with isosorbide 5-mononitrate after relieving shocks.Results The main symptoms of palinless AMI in patients with type 2 diabetes include chest distress,cardio-palmus,dyspnea,fatigue,hypertension,nausea,vomiting,cardiac arrththmia,heart failure and cardiac shock.AMI occurred in 6 cases of hypoglycemia.There existed significant difference in curative effects between the cases of cardiac shock group and the non-cardiac group(P<0.01).Conclusion Hypoglycemia is a major factor in inducing the disease.Good control of diabetes,Prevention of hypoglycemia,timely confirmation of painless AMI and early use of isosorbide 5-mononitrate can help to improve the prognosis.
