胰源性区域性门静脉高压的诊治分析

目的:探讨胰源性区域性门脉高压的临床特点和主要治疗措施。方法:收集1998～2010年收治的21例胰源性门脉高压患者临床资料,进行回顾性分析。结果:21例患者其中急性胰腺炎并发胰源性门脉高压6例,慢性胰腺炎10例,胰腺癌4例,胰腺外伤术后并发门静脉高压1例。15例行胃镜检查的患者中,11例有胃底静脉曲张(73.3%),3例合并食管静脉曲张。12例(57.1%)发生上消化道出血,13例行脾切除术。结论:胰腺炎和胰腺癌是引起胰源性门脉高压的主要原因,脾切除术是有症状胰源性门脉高压的主要治疗方法。

Analysis of Pancreatic Regional Portal Hypertension Diagnosis and Treatment

Objective. To investigate the clinic feature of pancreatic regional portal hypertension and main treatment methods. Methods： The clinic data of 21 cases of pancreatogenic portal hypertension, who were treated in our hospital from 1998 to 2010,were analyzed retrospectively. Results.6 cases were acute pancreatitis , 10 were chronic pancreatitis, 4 pancreatic cancer and 1case was pancreatic trauma after operation. 15 cases received gastric endoscopy, 11 cases had gastric fundus vein varices, 12 cases（57.1%） with upper gastrointestinal hemorrhage. 13 cases underwent splenectomy. Conclusions： Pancreatitis and pancreatic cancer are main causes of pancreatogenic regional portal hypertension. Splenectomy is the main treatment for symptomatic patients.
