后路椎间融合钉棒系统内固定与传统手术治疗腰椎间盘突出症的临床对比分析

目的 比较分析后路椎间融合钉棒系统内固定术与传统手术治疗腰椎间盘突出症的临床疗效及优缺点.方法 联合治疗组48例腰椎间盘突出症采用后路椎间融合器(PLIF)并椎弓根螺钉系统进行治疗；常规治疗组60例腰椎间盘突出症以传统常规开窗手术治疗.比较2组手术时间、出血量、住院时间;根据手术前后X线片、CT检查、Macnab评分,判定该2种术式的临床效果.结果 联合治疗组手术时间,术中出血量高于常规治疗组,差别有统计学意义(P＜0.05)；而联合治疗组的术后总体疗效明显高于常规手术组,同时住院时间少于常规治疗组,均有统计学意义( P＜0.05).结论 后路椎间融合钉棒系统内固定术治疗老年椎间盘突出症可以提高优良率和骨性融合率,明显改善临床症状.

Comparison of clinical efficacy between the treatment of posterior interbody fusion combined with intervertebral fixation and conventio- nal surgery in lumbar disc herniation

Objective To compare the efficacy of the posterior interbody fusion combined with intervertebral fixation and conventional surgery in treatment of lumbar disc herniation.Methods Posterior interbody fusion combined with intervertebral fixation and conventional surgery was performed in 48 and 60 patients with lumbar disc herniation,respectively.The operative time,blood loss and hospital stay was compared between the two groups;the fusion rate and clinical efficacy was evaluated according to postoperative X-rays、CT、Macnab criterion.Results There were no significant statistical difference in the issue of operative time,blood loss,fusion rate（P〉 0.05）between 2 groups;However,postoperative clinical efficacy was significantly higher in the combine treatment surgery group than in the conventional group（P〈 0.05）,meanwhile the hospital stay was also significantly less compare with the combine conventional group（ P〈 0.05）.Conclusion The treatment of posterior interbody fusion combined with intervertebral fixation can improve the Macnab Score and fusion rate,alleviating the clinical symptoms in the lumbar disc herniation of the elderly.
