胰岛素治疗糖尿病合并肺结核的临床观察

目的观察胰岛素治疗糖尿病合并肺结核的疗效。方法选择我院糖尿病合并肺结核患者35例,根据患者的依从性随机分成2组,胰岛素组19例,口服降糖药组16例。2组降糖药不同,抗结核药相同,初治病例应用3HRZE/6-9HRE方案,复治病例应用3HRZE/9-12HRE方案,同时2组均给予左氧氟沙星静脉滴注1个月,观察1个月、3个月的疗效。结果胰岛素组的临床症状改善、胸片转归及血糖水平明显优于口服药组。结论糖尿病合并肺结核患者最佳降糖方案为胰岛素强化治疗,而肺结核化疗疗效与糖尿病控制情况密切相关。

Efficacy of insulin in the treatment of diabetes mellitus complicated with pulmonary tuberculosis

Objective To observe the efficacy of insulin in the treatment of diabetes mellitus complicated with pulmonary tuberculosis.Methods 35 patients in our hospital with diabetes mellitus complicated with pulmonary tuberculosis were randomly divided into 2 groups according to the patients′ compliance:19 cases of insulin group and 16 cases of oral antidiabetic drugs group.The two groups were given the same anti-tuberculosis drugs and different antidiabetic drugs.The first treatment cases were given 3HRZE / 6-9HRE,and re-treatment cases were given 3HRZE / 9-12HRE.The two groups were given levofloxacin intravenously for 1 months,and all cases were observed for 1 and 3 months.Results The improve of clinical symptoms,chest X-ray and blood glucose level of insulin group were significantly better than those of oral antidiabetic drugs group.Conclusion The best regimen of diabetes patients complicated with pulmonary tuberculosis is intensive insulin therapy,and chemotherapy efficacy of pulmonary tuberculosis is closely related to diabetes conrtol.
