下腔静脉滤器置放术相关并发症初探

目的初步探讨下肢深静脉血栓形成患者行下腔静脉滤器置入术后的相关并发症。方法我院自1998年3月到2005年6月,33例经彩色多普勒超声确诊下肢深静脉血栓(deep vein thrombosis,DVT)患者置入下腔静脉滤器(vena cava filter, VCF),术后常规抗凝治疗,随访23例。结果本组VCF均成功植入,下腔静脉滤器放置部位血栓形成2例,滤器倾斜3例,右股静脉血栓形成1例,未发生肺栓塞(pulmonary embolism,PE)病例。结论滤器置入预防肺栓塞安全可行,严格掌握适应症、术中仔细操作、以及术后严格的抗凝治疗可避免或减少并发症的发生。

An exploration on complications following the placement of inferior vena cava filters

Objective To explore the causes of and preventive measures for complications following the placement of inferior versa cava filter ( VCF). Methods A total of 33 patients, with proven diannosis of lower limb deep vein thrombosis ( DVT) by Doppler ultrasononraphy, underwent the placement of permanent VCF from March 1998 to June 2005. Anticoagulation treatments were used in all the cases post - VCF insertion. Results The placement of VCF was successfully accomplished in all die cases in the study. Follow - up observations for 1-35 months revealed vena cava thrombosis at the site of insertion in 2 cases and filter tilting less than 150 in 3 cases and access vein thrombosis in 1 case. Conclusion Careful surgical managements and strict anticoagulation therapy are the key to the successful VCF' s placement and the prevention of PE or other complications.
