强直性脊柱炎全髋关节置换术临床分析

目的观察强直性脊柱炎(AS)合并髋关节强直或疼痛患者行全髋关节置换术的疗效。方法对48例(77髋)行全髋关节置换术,术后随访12~96个月。结果患者术前髋关节总活动度为36.4°±8.8°,术后为193.0°±12.7°。术后无脱位、感染等并发症。Harris评分术前平均为(33.0±9.0)分,术后为(85.3±7.4)分,其中优55髋,良13髋,可8髋,差1髋,优良率88.3%。X线片显示未见假体松动,7髋(9.1%)发生异位骨化。结论全髋关节置换术治疗AS髋关节病变临床效果满意。

Total hip arthroplasty for treatment of ankylosing spondylitis

Objective To observe the curative effect of total hip arthroplasty （THA） for treatment of patients with ankylosing spondylitis（AS） complicating arthrocleisis. Methods Forty-eight patients （77 hips） with AS were performed with THA,all cases were followed-up for 12 -96 months after operation. Results The total range of motion improved from 36.4° ± 8.8° before surgery to 193.0°±12.7° at the final follow-up time. No complication such as dislocatlon,infection happened. The average Harris hip score increased from 33.0 ± 9.0 points before operation to 85.3 ± 7.4 points after operation. According to Harris hip score,55 hips got excellent results, 13 hips got good results and 9 hips got fair results,the good and excellent rat was 88.3%. Aceording to X ray, there were no loosening of prosthesis, heterotopic ossification appeared in 7 hips （9. 1% ）. Conclusion Total hip arthroplasty is a very important and effective treatment to patients with AS.
