电视胸腔镜手术诊治胸部疾病

目的探讨电视胸腔镜手术(VATS)诊治常见胸外科疾病的手术方法及效果.方法 1994年4月～2003年9月施行VATS诊治胸部疾病277例,其中自发性气胸、血气胸187例,孤立性肺结节51例,胸腔积液4例,局限性胸膜间皮瘤、脓胸及凝固性血胸各3例,中段食管癌5例,重症肌无力21例.结果所有病例均得以明确诊断或完成治疗.全组无手术死亡及危象发生.手术时间15～140min,平均54.7min,术中出血10～200ml,平均69.3ml;住院1～20d,平均9.2d.结论 VATS选择性诊治胸外科疾病安全有效,具有明显的微创优势.

Diagnosis and treatment of chest diseases with video-assisted thoracoscopic surgery

Objective To study the techniques and effects of video assisted thoracoscopic surgery(VATS) in diagnosis and treatment of the common chest diseases.Methods From April 1994 to September 2003,277 patients with chest diseases receiued VATS.And the diseases included 187 spontaneous pneumothorax of hemopneumothorax,51 solitary pulmonary nodule,4 pleural effusion,3 localized benign mesothelioma,3 empyema,3 coagulated hemothorax,5 esophageal carcinoma and 21 myasthenia gravis.Results All the diagnosis or treatments were successfully carried out and no death and myasthenia crisis associated with operative procedure were found.The mean operative time was 54.7min(15 140min).The mean operative blood loss was 69.3ml(10 200ml).And the mean postoperative hospitalized time was 9.2d(1 20d).Conclusion To the selected patients,VATS is a minimally invasive,safe and effective technique in the diagnosis and treatment of chest diseases.
