摘    要:目的探讨经单侧半椎板入路在显微切除椎管肿瘤中的应用.方法报道经单侧半椎板切除21例椎管内肿瘤,肿瘤均位于髓外硬膜下,其中神经鞘瘤14例,脊膜瘤7例.肿瘤位于颈段8例,胸段4例,腰段9例.结果所有肿瘤均获全切,术后当时、1周和随访期内(2~36个月)症状和体征均明显改善,无手术并发症和脊柱畸形.结论经单侧半椎板入路可最大程度暴露脊柱后的肌肉韧带和骨结构,且手术创伤小、安全,在一定程度上优于全椎板切除入路.

Abstract:s Objective To evaluate the application of hemilaminectomy in the microsurgical removal of the intraspinal tumors. Methods We reported 21 patients with intradural extramedullary tumors undergone surgery through unilateral hemilaminectomy, including 14 schwannomas and 7 meningiomas. In which 8 were cervical, 4 were thoracic, and 9 lumbar. Results All the tumors were totally removed. The symptoms and signs of the patients improved remarkably in the postoperative evaluations performed immediately after the surgery, one week later, and during follow-up period (2m-36m). None of the patients showed spinal deformity or spinal instability. Conclusion The unilateral hemilaminectomy could preserve the musculoligamentous attachments and bony posterior elements as much as possible. It is a minimally invasive and safe surgery and superior to the whole laminectomy in some ways.
