胃癌低发区30例进展期胃癌临床病理观察

本文对胃癌低发区30例进展期胃癌的临床和病理特点进行了观察,结果表明:①患者男女之比为9∶1,临床病程短,癌平均发病年龄51岁,临床症状多不典型;②胃癌组织类型,肉眼多为浸润溃疡型,显微镜观察癌溃疡四层结构不典型,底部常有肌残留和癌浸润,类型以管状腺癌和低分化腺癌多见;③胃癌癌旁伴发病变(CAG、DYS、ⅠM、Ⅱb)检出率均明显高于同例非癌旁相应病变(P<0.01);④粘液组化(HID/AB)染色证实,14例胃癌癌旁有含硫酸粘液结肠肠化(Ⅱb型肠化),表明Ⅱb型肠化与胃癌发生关系更密切。

THE CLINICO-PATHOLOGICAL OBSERVATION ON 30 CASES WITH ADVANCED GASTRIC CARCINOMA (AGC) IN LOW-INCIDENCE REGIONS

Thirty cases of AGC in low incidence regions were studied clinically and pathologically. The results show that:(1)Males are more often affected than females, with the ratio of 9 to 1; the average age of AGC is 51, the clinica symptoms are usually not typical(2)Macroscopically, the type of AGC is often the infiltrative ulcerated pattern; microscopically, the four layer struc-tures of ulcer base are often not typical either, of which the muscle residue and cancer infiltration are often found, the tubular adenocareinoma and poorly differentiated carcinoma occure commonly; (3)The detectable rates of CAG, dys Im IIb Im lesions are remarkably higher in sections adjacent to cancer thanin sections not adjacent, their difference being statistically significant(P<0.01). (4)14 cases with AGC have the pattern of IIb intestinal metaplasia, which suggests that the IIb Im is closely related to the pathogenesis of gastric carcinoma.
