难治性肺结核与初治性肺结核耐药性测定

目的通过耐药性测定为临床化疗方案及控制肺结核流行制定防治对策提供科学依据。方法对308例难治性长期排菌的慢性肺结核及初治菌阳肺结核对照组202例进行耐药性测定。取晨起第一口疾,以2％硫酸作预处理,30分钟后取液化痰没0.1ml接种于改良的罗氏培养基上。结果难治性肺结核耐药的发生率明显高于初治肺结核,不同浓度的INH、RFP、EMB和SM耐药率也有显著性差异(P＜0.01)。结论无论难治性或初治的肺结核,均以耐SM为首位,同时对INH、SM、RFP及EMB的耐药率均较高,。尤具RFP的初始耐药率高达27.2％。

The measurement of drug resistance in intractable pulmonary tuberculosis and initial treated pulmonary tuberculosis

Objictivc Tlie aim of this study was to provide scientific basis for chemotherapeutical.schemes, and for protection and treatment policies in controlling epidemic of tuberculosis. Methods Measurement of drug resistance in 308 cases of chronic tuberculosis with discharging bacteria for a long time, and in 202 cases of pulmonary tuberculosis in initial treament with positive tubercle bacillus in sputum as control group, was undertaken. The first sputum in the early morning was taken and pretreated with 0.2% sulfuric acid. After 30 minutes, 0. 1ml liquefactive sputum was taken and inoculated on Rosenbach's culture medium contained drugs. Results The rate of drug resistance of intractable pulmonary tuberculosis is obviously higher than that of initial treatment pulmonary tuberculosis, and there was a significance difference in different concentrition of INH, REP,EMB, and SM (P＜0.01)Conclusion The drug toleration to SM was the first in both intractable and initial treated pulmonary tuberculosis. At the same time, there is a higher rate of drug resistance to SM, INH, RFP and EMB in both the intractable and initial treated pulmonary tuberculosis, especially the rate of initial drug resistance to RFP is higher up to 27. 1 %.
