摘    要:目的 研究趋化因子CCL2单核细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)]、CCL3巨噬细胞炎性反应蛋白-1α(macrophage inflammation protein 1α,MIP-1α)]、CCL4(MIP-1b)、CCL5(RANTES)、Eotaxin在慢性鼻窦炎(chronic rhinosinusitis,CRS)组织中的表达并观察上述趋化因子与炎性细胞标志物和其他细胞因子的相关性.方法选择94例鼻息肉组织(chronic rhinosinusitis with nasal polyps,CRSwNP),33例慢性鼻窦炎组织(chronic rhinosinusitis without nasal polyps,CRSsNP)和24例对照鼻甲黏膜.采用组织匀浆,ELISA检测CCL2(MCP-1)、CCL3(MIP-1a)、CCL4(MIP-1b)、CCL5(RANTES)和Eotaxin,同时采用ELISA检测组织中的嗜酸性阳离子蛋白(eosinophilic cationic protein,ECP)、髓过氧化物酶(MPO)、白细胞介素-5(IL-5)、肿瘤坏死因子(tumor necrosis factor-α,TNF-α)、IL-6、IL-8.结果 CCL2(MCP-1)、CCL3(MIP-1a)、CCL4(MIP-1b)和Eotaxin在鼻息肉组织(chronic rhinosinusitis with nasal polyps,CRSwNP)中的表达高于对照组和鼻窦黏膜组织(chronic rhinosinusitis without nasal polyps,CRSsNP),并且CCL4和Eotaxin在嗜酸性粒细胞型鼻息肉(Eos CRSwNP)组织中的表达明显高于中性粒细胞型鼻息肉(Neu CRSwNP)组织.而CCL5(RANTES)在各组间差异无统计学意义.结论趋化因子在鼻息肉发病机制中可能发挥重要作用.

Abstract:Objective To evaluate the expression and roles of ehemokines in chronic rhinosinusitis without nasal polyps (CRSsNP) and with nasal polyps(CRSwNP) in Northern China. Methods Ninety-four cases of NP tissue, 33 cases of sinus mueosa of CRSsNP and 24 eases of control were enrolled in the present study. ELISA was used to measure CCL2( MCP-1 ), CCL3 (MIP-la), CCIA( MIP-lb), CCL5 (RANTES) and Eotaxin in homogenization of nasal mueosa and polyp tissue. Results CCL2 (MCP-1) , CCL3 (MIP-1 a) , CCIA (MIP-lb) and Eotaxin in NP tissue were significantly higher than those in CRSsNP and control tissue. CCIA(MIP-lb) and Eotaxin in eosinophilie NP tissue was significantly higher than those in neutrophilic NP. No signifieant difference of CCL5 (RANTES) was found among the above groups. Conclusion Chemokine CCL2(MCP-1 ), CCL3 (MIP-la), CCL4(MIP-lb) and Eotaxin may play important roles in the pathogenesis of nasal polyps.
