大鼠应激性胃粘膜损伤与壁细胞泌酸的关系

摘    要:目的:探讨应激状态下大鼠胃粘膜损伤与壁细胞泌酸状态的关系。方法:将48只SD大鼠随机分成对照组、水浸束缚应激1、2、4h组、奥美拉唑组及生理盐水组。检测胃液pH值、胃粘膜损伤指数(UI),光镜观察胃粘膜组织学改变,电镜观察壁细胞超微结构变化。结果:随应激时间延长UI明显增加,而胃液pH值明显降低(P<0.01),二者之间呈明显负相关(γ=-0.9987,P<0.01);奥美拉唑组与应激4h组和生理盐水组比,UI明显降低(P<0.01)。电镜示壁细胞在对照组呈静息状态,而应激1h,2h,4h组呈不同激活状态,以4h最明显,呈泌酸活跃状态。奥美拉唑组壁细胞见分泌小管扩张,有绒毛稀少或密集分布的不同表现。结论:应激状态下大鼠胃粘膜损伤程度与壁细胞泌酸状态密切相关。

Abstract:Objective To observe the relationship of gastric mucosal lesions and the oxyntic function of parietal cells in rats under stress Methods Forty eight male SD rats were randomly divided into 6 groups: control group, 1 h, 2 h, 4 h groups under water restraint stress (WRS), omeprazole treated group and normal saline treated group (NS) The gastric fluid pH value and gastric mucosal ulcer index (UI) were measured The changes of gastric mucosal lesions were observed with light microscopy, and the ultrastructural changes of parietal cells were observed with transmission electronic microscopy (TEM) Results With the stress time prolonged, the UI increased and the pH value decreased gradually and significantly under WRS compared with control group ( P <0 01) Moreover, there was a negative relationship between UI and pH value( r =-0 9987) The UI of omeprazole treated group decreased significantly compared with stress 4 h group and normal saline treated group ( P <0 01) Parietal cells were in a resting state in control group, but became active in stress 1, 2, 4 h groups, and especially stress 4 h group in an active secreting state The dilated intracellular canaliculi lined with numerous or rare microvilli were observed in omeprazole treated group under TEM Conclusion The results indicate that the degree of gastric mucosal lesion is closely correlated with the oxyntic function of gastric parietal cells in rats under stress
