左侧区域性门静脉高压症临床分析

目的分析左侧门静脉高压症的病因、内镜下特征、误诊原因及治疗方法。方法回顾分析26例手术证实的左侧区域性门静脉高压症患者的临床资料。结果左侧区域性门静脉高压症主要临床表现为呕血和（或）黑便，脾脏肿大、脾功能亢进，多有胰腺痰患而无肝脏痰病。超声提示脾静脉阻塞或扩张，胃镜可见胃底孤立静脉曲张。该病少见，易误诊。结论孤立性胃底静脉曲张是左侧区域性门静脉高压较特征的表现之一，首选手术治疗。

Clinical Analysis of Portal Hypertension of the Left Part

Objective To study the cause of portal hypertension on the left part,its features under endoscope,the misdiagnosis and its treatment.Methods A retrospective analysis was made to the clinical data of 26 cases with portal hypertension on the left part proved by operation.Results The main symptoms were hematemesis,melena,splenomegaly and hypersplenism,most of the patients suffered from pancreatic diseases,but they didn't have hepatic diseases.Ultrasound examination indicated the obsrtuction or dilation of pancreatic vein,the gastric endoscope examination showed isolated gastric varices.Conclusions lsolated gastric varices are one of the typical symptoms of portal hypertension on the left part,and surgical treatment would be the first choice.
