86例原发性腹膜后肿瘤的诊疗分析

目的探讨原发性腹膜后肿瘤的诊断和手术治疗方法。方法回顾性分析我院1999年至2008年间经手术和病理证实的86例原发性腹膜后肿瘤临床资料。结果 B超、CT、MRI、DSA对腹膜后肿瘤诊断的正确率分别为73.08%、90.31%、100%1、00%;86例患者均行手术治疗,肿瘤全切66例（76.74%）,全切者术后2、5、10年的平均存活率分别为84.23%、62.56%、51.45%,部分切除者术后2、5、10年的平均存活率分别为37.21%、21.58%、11.34%;86例病理结果为良性肿瘤34例（39.53%）,恶性肿瘤52例（60.47%）。结论 B超和CT可以作为诊断原发性腹膜后肿瘤的主要检查方法 ,手术切除仍是原发性腹膜后肿瘤的首选治疗手段,手术效果与肿瘤能否全切直接相关,完全切除者术后的平均存活率明显高于部分切除者。

Analysis of diagnosis and surgical treatment of 86 cases with primary retroperitoneal tumor

Objective To explore the diagnosis and surgical treatment of primary retroperitoneal tumor（PRT）.Methods 86 cases with PRT treated in Chongqing tumor hospital from 1999 to 2008 were analysed in the clinical manifestations,imaging findings,pathological types and operative resections.Results The accuracy rates of B-ultrasonic,CT,MRI,DSA were 73.08%,90.31%,100%,100%,respectively.All of 86 cases were performed operation,tumors of 66 cases（76.74%） were completely reseceted.Among them,30 cases（88.23%） were benign tumors and 36 cases（69.23%） were malignant tumors.Average survival rates of cases with complete surgical resection in 2 years,5 years and 10 years post-operation were 84.23%,62.56%,51.45%.Accordingly,cases with partial surgical resection were 37.21%,21.58%,11.34%.Among 86 cases,34 cases（39.53%） are benign tumors,52 cases（60.47%） are maglinant tumors.Conclusions B-ultrasonic and CT examination could be used as main methods for diagnosis of PRT,surgical resection is the leading treatment method for PRT,complete resection of tumor is the key factor for survival rate and life quality of PRT.
