安氏Ⅱ~1类错畸形矫治前后颅颌面部软硬组织变化的研究

目的研究采用直丝弓固定矫治的恒牙期青少年安氏Ⅱ1类错畸形患者矫治前后颅颌面部软硬组织的结构变化。方法随机选择安氏Ⅱ1类病例32例,分为拔牙组与非拔牙组,各16例。采用头影测量软件测量其矫治前后头颅定位侧位片（共含21个测量项目）,所得数据进行统计学分析。结果矫治前后的变化主要为覆合覆盖减小,牙轴改变和磨牙关系纠正以及软组织侧貌改善。拔牙组覆盖减小主要为上切牙舌倾实现,非拔牙组则为上切牙舌倾和下切牙唇倾的共同结果;垂直向软硬组织变化无统计学意义;软组织的变化主要为鼻唇角增大和上下唇相对于审美平面的位置变化。结论直丝弓矫治恒牙期青少年安氏Ⅱ1类错畸形可改变牙齿位置及软组织侧貌,对骨骼结构无明显作用。

A study of the changes in the craniomaxillofacial soft and hard tissues following orthodontic treatment in patients with Angle Class II Division I malocclusion

Objective To investigate the changes in the craniomaxillofacial soft and hard tissues following orthodontic treatment with straight-wire appliance in adolescents of permanent dentition with Angle Class Ⅱ Division Ⅰ malocclusion.Method Thirty-two classⅡ divisionⅠ cases were randomly chosen and divided into two groups by extraction and non-extraction.Soft and hard tissue changes of class Ⅱ division Ⅰ cases after orthodontic treatment is analyzed with Cephalometric Radiography.The results were statistically analyzed.Results The changes following the orthodontic treatment included decrease in anterior overjet and overbite,changes in tooth axis and the correction of molar relationship as well as improvement of lateral view of soft tissues.The decrease in overjet was achieved by lingual inclination of upper incisors in the extraction group,while in the non-extraction group it was realized by the combination of lingual inclination of upper incisors and labial inclination of lower incisors;there was no statistical difference in the changes in vertical soft tissues;the changes in vertical dimension of soft tissues mainly included enlargement of nasolabial angle and changes in lip positions on aesthetic plane.Conclusion In the treatment of Angle Class Ⅱ Division Ⅰ malocclusion in the adolescent patients of permanent dentition,the straight-wire appliance can change the position of the teeth and lateral view of the soft tissues,and has no evident effect on the skeletal pattern.
