青老年股骨颈骨折内固定生物力学研究

摘    要:目的 比较3种常用内固定方法对股骨颈骨折的固定效果。方法 将24具新鲜股骨颈骨折标本按年龄分为青、老年2组,采用双曝光散斑干涉法和加载目测法进行生物力学测定。结果 在股骨颈骨折内固定中,多针在对抗骨折面间旋转和剪力方面效果较好,而加压螺纹钉在对抗分离、剪力及旋转方面具有更好的固定效果。多针内固定主要适合于青年人,而加压螺纹钉内固定既适合于青年人,又适合于老年人。结论 多针内固定比加压螺纹钉内固定更具优势。

Biomechanical Study on Fixators of Femoral Neck Fracture in Young and Old Patients
Abstract:Objective To test the efficacy of three commonly used internal fixation methods of osteosynthesis in femoral neck fracture. Methods 24specimens of femoral neck fracture, which were produced on young and old adult fresh femoral samples were divided into two groups according to ages. Using double exposure of laser spoke photography and double exposure of laser holography techniques and macroscopy to measure the proximal part of the femur. Results The biomechanical measurement data showed that fixation with multiple pins on the femoral neck fracture was firm in withstanding rotation and shear displacement, but the compressing screw fixing group was more stable in withstanding separation and shear displacement and torsion. The multiple pins was chiefly fit to young patients and the compressing screw fixing group was not only fit to young but also to old patients. Conclusion The fixation with compressing screw was superior to that with multiple pins.
