经皮冠状动脉介入治疗对急性冠脉综合征近期预后的影响

目的探讨经皮冠状动脉介入治疗(PCI)对急性冠脉综合征(ACS)心功能变化、左心室重构、运动耐量、心绞痛发作及1年心因性病死率的预测价值。方法将我院心内科2004年10月～2005年10月住院的ACS患者根据是否行PCI治疗分为PCI组51例和非PCI组63例,比较两组患者心功能、左心室重构、运动耐量、心绞痛发作及1年心因性病死率。结果PCI组左心室舒张末期内径,左心室舒张末期容积,左心室收缩末期容积均较非PCI组明显减小(P<0.05),运动耐量较非PCI组明显增加(P<0.01),心绞痛发作次数较非PCI组明显减少(P<0.01),左心室射血分数较非PCI组明显增加(P<0.01),1年心因性病死率,PCI组为2.0%,非PCI组为9.5%,差异有统计学意义(P<0.05)。结论PCI治疗可改善ACS患者近期预后,1年心因性病死危险减少。

Effect of Percutaneous coronary intervention(PCI) on the short -term prognosis of acute coronary syndrome

Objective To assess the predictive value of Percutaneous coronary intervention(PCI) on dysfunction and left ventricular remodeling in patients with acute coronary syndrome(ACS).Methods The patients with acute coronary syndrome in hospital in 2004.10~2005.10 were divided into two groups according to the PCI:PCI group(n=51) and non PCI group(n=63).Left ventricular dysfunction and Left ventricular remodeling and mortality in 1 year were compared between the two groups.Results End-diastolic left ventricular internal diameter,end-diastolic left ventricular volume and end-systolic left ventricular were decreased in PCI groups compared with non PCI group(P<0.05,P<0.01).Exercise endurance test were increased in PCI groups compared with non PCI group(P<0.01).Angina times were decreased in PCI groups compared with non PCI group(P<0.01).Left ventricular ejection fraction was increased in PCI groups compared with non PCI group(P<0.01).The mortality in 1 year was 2.0% in PCI group,9.5% in non PCI group.A significantly different mortality was found between two group(P<0.05).Conclusion PCI can improve the patients prognosis with ACS.The mortality in 1 year decreased in the patients of PCI OF ACS.
