Abstract:Objective To compare the therapeutic effects of three methods on bupivacaine-induced toxicities in rats, and explore a safe and effective treatment method on the toxicities of local anesthetic. Methods Thirty-six male SD rats were randomly assigned to three groups (n=12 each): propofol group (group P), midazolam group (group M) and propofol and midazolam group (group PM). Lead Ⅱ of electrocardiogram (ECG) was continuously monitored. The femoral artery was cannulated for direct measurement of arterial blood pressure. The femoral vein was cannulated for infusion of 0.5% bupivacaine at the rate of 2 mg/(kg·min) until mean arterial blood pressure (MAP) decreased to 5.3 kPa or less. And then within 30 seconds, group P was infused of propofol 50 mg/kg, group M of midazolam 5 mg/kg and group PM of propofol 25 mg/kg and midazolam 2.5mg/kg. The infused volume was 5ml/kg. Results There were no significant differences among three groups in the occurrence time of toxicities reaction including arrhythmias, convulsion and MAP≤5.3kPa, and the total dosage of bupivacaine. MAP of group PM was significantly higher than that of group P at 1, 3 and 5 minutes, and higher than that of group M at 3 and 5 minutes after resuscitation treatment (P0.05). The survival rate was significantly increased in group PM (P0.05). Conclusion Propofol combined with midazolam has a synergistic therapeutic efficacy on severe bupivacaine-induced toxicities in rats, and can reduce the side effects of drugs and improve the survival rate of rats.
