原发性肝细胞癌中mdr_1基因表达的研究

目的　研究多药耐药基因mdr1产物P 糖蛋白 (Pgp)在原发性肝细胞癌 (PHC)及其癌旁组织中的表达 ,探讨PHC的多药耐药机制。方法　应用链菌素亲生物素 过氧化物酶标免疫组织化学染色 (S P)法观察 2 4例未化疗、36例化疗后PHC及其癌旁组织Pgp的表达 ,2 0例门脉性肝硬化、2 0例正常肝组织的Pgp表达。 结果　肝动脉插管化疗栓塞 (TACE)前、后的PHC与非癌肝组织Pgp的表达为 6 6 .6 7%、80 .5 6 %与 90 .0 0 % ,相互间差异无显著性 (P >0 .0 5 ) ,TACE前、后的癌旁肝组织全部表达Pgp ;PHC的Pgp表达可强于、弱于或相近于其癌旁组织 ,PHC的Pgp表达与病理组织学类型、癌细胞分化程度无明显相关。结论　PHC的多药耐药性源于内源性的Pgp表达 ,化疗药物的诱导可能不起作用。

Study of expression of mdr_1 gene in primary hepatocellular carcinoma

Objective To study expression of mdr 1 gene product P-glycoprotein (P-gp) in primary hepatocelular carcinoma (PHC) and its adjacent liver tissues, analyze the mechanism of multidrug resistance (MDR) of PHC. Methods Using immunohistochemical S-P method, expression of P-gp was detected in 24 untreated PHC and their adjacent liver tissue, 36 treated PHC and their adjacent liver tissue, 20 portal cirrhosis tissues and 20 normal liver tissues. Results Expression of Pgp in untreated PHC, treated PHC and non-malignant liver tissue is 66.67%, 80.56% and 90.00% respectively, without significant difference in untreated PHC, treated PHC and non-malignant liver tissue. All untreated and treated tumor adjacent liver tissues expressed Pgp. Pgp expression in PHC may be stronger, lesser than or similar to that in its tumor adjacent liver tissue. Pgp expression in PHC was not correlated with histological type and differentiation. Conclusion The MDR of PHC is derived from the intrinic MDR and inducement of chemotherapeutic agen has little effect.;
