雷贝拉唑或奥美拉唑三联疗法治疗HP阳性消化性溃疡疗效的Meta分析

目的对以雷贝拉唑或奥美拉唑为基础的一周三联疗法治疗幽门螺杆菌（HP）阳性消化性溃疡的疗效进行Meta分析,以客观评价其疗效。方法搜索2000年10月至2008年10月国内期刊公开发表的关于比较雷贝拉唑与奥美拉唑三联疗法治疗HP阳性消化性溃疡的随机对照试验或临床对照试验,根据标准筛选出9篇文献,运用RevMan4.2统计软件对文献试验数据进行Meta分析,绘制森林图,同时绘制漏斗图,检查发表偏倚。结果雷贝拉唑三联疗法的1,3,7d时症状缓解情况及HP根除率、溃疡愈合率显著优于奥美拉唑三联疗法;在治疗溃疡有效率及不良反应发生率方面,两种方案无显著性差异。结论与奥美拉唑三联疗法比较,雷贝拉唑三联疗法具有起效迅速、临床症状缓解快、HP根除率高、溃疡愈合时间短等优点,是治疗HP阳性消化性溃疡的优选方案。

A Meta Analysis of Efficacy Comparison of Rabeprazole and Omeprazole Based Triple Therapy in Treating Peptic Ulcer in Helicobacter Pylori Positive Patients

Objective To evaluate the efficacy of rabeprazole or omeprazole based one week triple therapy in treating peptic ulcer in the Helicobacter pylori （HP） positive patients. Methods To retrieve the published domestic documents on randomized controlled trials or controlled clinical trials comparing rabeprazole with omeprazole based triple therapy in treating peptic ulcer in HP positive patients. According to criterion 9 documents were selected. The forest plot with meta analysis was drawn by RevMan 4.2 statistical software from the documents data and the funnel plot was also drawn to check the literature publication bias. Results Rabeprazole based triple therapy was superior to omeprazole based triple therapy in the aspects of symptomatic relief after 1, 3, 7 d-treatment, eradication rate of HP and cure rate of ulcer. There were no significant differences in the treatment efficacy of ulcer and the incidence of adverse reactions. Conclusion Rabeprazole based triple therapy have advantages of instant effect, easing symptoms quickly, high HP eradication rate and less time for ulcer cure over omeprazole based triple therapy. So the rabeprazole based triple therapy is the better choice for the treatment of peptic ulcer in the HP positive patients.
