联合应用B超及血清学检测对诊断早期异位妊娠的价值

目的联合检测子宫内膜厚度、血清β-HCG及孕酮以早期诊断异位妊娠,防止临床误诊。方法对138例疑是异位妊娠的患者测定血清β-HCG、孕酮及经阴道超声检测子宫内膜厚度。按最终诊断分3组:A组为异位妊娠(早期未破裂者)78例;B组为宫内妊娠流产35例;C组为宫内正常妊娠25例。结果C组的子宫内膜厚度、β-HCG、孕酮值均大于B组及A组(P均0.01)。B组的β-HCG值与A组相比有显著性差异(P0.01),子宫内膜厚度、孕酮相比均无显著性差异(P均0.05)。结论子宫内膜厚度、血清β-HCG及孕酮均可作为诊断异位妊娠的指标,但三者联合检测可以预测妊娠的着床部位,在防止误诊中更具有价值。

Value of type-B ultrasonic combined with serology detection in diagnosing early ectopic pregnancy

Objective It is to united detect endometrial thickness,β-HCG level and progesterone level to early diagnose ectopic pregnancy and to avoid clinical misdiagnosis.Methods The endometrial thickness,serum β-HCG and progesterone level were measured in 138 suspicions ectopic pregnancy patients.They were divided into three groups on the basis of their diagnoses.78 patients with ectopic pregnancy were in group A,35 patients with abortion after pregnancy-in-uterus were in group B and 25 patients with normal pregnancy in uterus were in group C.Results The endometrial thickness,β-HCG level and progesterone level in group C were significantly bigger than those in group A and group B(all P<0.01).There was significant difference on the level of β-HCG between group B and group A(P<0.01),but there was no significant differences on the level of progesterone and the endometrial thickness between them(all P>0.05).Conclusion The determination of β-HCG and progesterone and the measurement of endometrial thickness all can be taken as the index of diagnosing ectopic pregnancy,but the three parameters united detection can prognosis gestational nidation position and have a diagnostic value in avoid misdiagnosis.
