肾移植术后近期发生高热的诊断及治疗

报道尸体肾移植后近期发生高热５７例。其中肺部感染３５例，占６１．４％；移植肾周围脓肿６例，占１０．５％；疟原虫感染４例，占７％；脓毒血症１例，占１．８％；肺脓肿１例，占１．８％，颅内感染１例，占１．８％，不明原因９例，本组治愈４１例，占７１．９％，摘除移植肾３例，占５．３％，死亡１３例，占２２．８％。就近期发热的原因，鉴别诊断，治疗及预防进行讨论。

Early postoperative high fever after cadaveric kidney transplantation

Early postoperative high fever after cadaveric kidney transplantaion occurred in 57 cases.It was due to lung infection in 35 (61. 4%),25 of them being bacterial infection,5 cytomegalovirus,3 tuberculosis and 2 fungal infection.Perinephritic abscess at the transplantation site occurred in 6 of the 57 (10. 5%),plasmodiosis in 4 (7%), 3 pyemia, 1 pulmonary abscess and 1 intracranial infection. The cause of the high fever has been unknown in 9 patients. Cure has been achieved in 41 (71. 9%) while the transplant has to be extirpated in 3 (5. 3%).13 patients (22. 8%) died of the complication. The cause,differential diagnosis,prevention and management of early postoperative high fever after cadaveric kidney transplantation were discussed.
