短期胰岛素泵强化治疗对初诊2型糖尿病患者氧化应激的影响

摘    要:目的探讨短期胰岛素泵强化治疗和多次皮下胰岛素注射对初诊2型糖尿病患者8-异前列腺素F2α和C反应蛋白的影响。方法初诊2型糖尿病患者60例随机分为两组,分别给予胰岛素泵(CSII组)和多次皮下胰岛素注射(MSII组)治疗2周。比较两组治疗前后空腹血糖(FPG)、餐后2h血糖(2hPG)、空腹C肽(FC-P)、餐后2hC肽(2hC-P)、糖化血红蛋白(HbA1C)、血8-异前列腺素F2α、C反应蛋白(CRP)等变化情况。结果MSII组和CSII组治疗后FPG、2hPG、HbAle、8-异前列腺素F2α、CRP均较治疗前明显下降,FC—P、2hC—P均较治疗前明显升高(P〈0.05)。CSII组FPG、2hPG、C-P、2hC-P、8-异前列腺素F2α、CRP治疗前后变化量明显高于MSII组(P〈0.05)。CSII组较MSII组胰岛素用量明显减少(P〈0.05)。结论与多次皮下胰岛素注射组相比,胰岛素泵短期强化治疗不仅具有平稳降糖、改善β细胞功能及胰岛素抵抗的作用,而且具有改善氧化应激及降低炎症的作用。

Effect of short-term insulin pump therapy on oxidative stress in patients with newly diagnosed type 2 diabetes

Abstract:Objective To investigate the effect of short-term insulin pump therapy and multiple subcutaneous injection of insulin on 8-iso-prostaglandin F2α and C-reactive protein in newly diagnosed type 2 diabetes patients. Methods Sixty patients newly diagnosed with type 2 diabetes were randomly divided into two groups. The patients were given insulin pump therapy in CSII group and multiple subcutaneous injection of insulin in MSII group for 2 weeks. Fasting plasma glucose (FPG), 2-hour postprandial blood glucose (2 hPG), fasting C-peptide (C-P) ,2-hour postprandial C-peptide ( 2 hC-P), glycated hemoglobin ( HbA1c), blood 8-iso-prostaglandin F2α, C-reactive protein(CRP) and other changes were compared before and after treatment. Results After treatment,FPG,2hPG,HbAlc, 8-iso-prostaglandin F2α, and CRP decreased significantly in two groups (P 〈 0.05 ), and FC-P,2 hCP were significantly increased (P 〈 0.05 ). The difference of FPG,2 hPG, CP,2 hC-P,8-iso-prostaglandin F2α, CRP before and after treatment were significantly higher in CSII group than that in MSII group (P 〈 0.05). The amount of insulin was significantly reduced in CISS group than that in MISS group. Conclusion Compared with multiple subcutaneous injection of insulin, short-term intensive insulin pump therapy not only keep stable blood glucose and improve β-cell function and insulin resistance, but also could improve the oxidative stress and reduce the inflammation.
