剖宫产221例患者应用抗菌药物的情况分析

目的了解剖宫产抗菌药物应用现状，为规范抗菌药物合理使用提供科学依据。方法回顾性分析2012—05—2012—12解放军总医院第一附属医院剖宫产手术病历。结果共调查病历221份，抗菌药物使用率为100％，给药途径均采用静脉滴注。联合用药率为93．67％，抗菌药物使用前3位为奥硝唑氯化钠注射液、注射用头孢呋辛钠、注射用乳糖酸阿奇霉素。用药疗程≤2d占0．90％，3d占3．62％，4d占83．71％，≥5d占11．76％。结论解放军总医院第一附属医院剖宫产患者围术期抗菌药物预防应用存在不合理之处，存在药物选用不合理、用药时间长、部分抗菌药联用指征过宽等问题，需进一步规范管理，促进抗菌药物合理使用。

Survey on application of antibiotics in 221 patients with cesarean section

Objective To investigate the application of antibiotics in patients with cesarean section for providing references for rational use of drugs. Methods Case reports of cesarean section form May to December 2012 were collected and analyzed. Results A total of 221 case reports were analyzed. All patients used antibiotics, and the way was intravenous drip. The combined use rate of antibiotics was 93.67%. The top three of the most frequently used antibiotics were ornidazole,cefuroxime sodium and azithromycin. The drug use ≤2 d acounted for 0.9% ,3 d for 3.62% ,4 d for 83.71%, ≥5 d for 11.76%. Conclusion There is irrational prophylactic use of antibiotics for cesarean section patients in perioperative period in First Affiliated Hospital of General Hospital of PLA. The three most important problems are the inappropriate drugs, long-term use and expansion of combined indication. It is necessary to strengthen the management and improve the rationality of application of antibiotics.
