乳腺癌术后胸壁复发溃疡的外科治疗

目的探讨乳腺癌术后胸壁癌复发溃疡的外科治疗方式。方法对26例乳腺癌术后胸壁癌复发溃疡病人，均行胸壁溃疡整块手术切除，并行皮瓣转移修补术，术后加用全身化疗、局部放疗及内分泌治疗。结果术后皮下积液4例，经抽吸治愈，5例出现周围皮缘坏死，经清创及再植皮痊愈。随访6～48个月，6、12、24和48个月存活率分别为92％、77％、62％和46％。结论乳腺癌术后胸壁复发溃疡采用手术切除、周围皮瓣转移术，能提高患者的生活质量，配合综合治疗。可延长存活率。

Effective surgical treatment for chest wall recrudesce ulcer for postoperative breast cancer

Objective] To explore the effective surgical treatment for chest wall recrudesce ulcer for postoperalive breast cancer. Methods] The 26 patients chest wall recrudesce ulcer for postoperative breast cancer, total applied chest wall ulcer excision, and process skin flap repair the ulcer wounds, and apply total chemical treatmen4 redioactive treatment and endocrine treatment. Result] The 4 patients of effusion under skin for postoperation are cured by drawing. The 5 patients has appeared necrosis in edage of skin, who are cured by debridement and skin allograft. The surival rate of the visited patients is 6-48 months. The rate of 6 month, 12 month, 24 month, 48 month is 92%, 77%, 62%, 46%. Conclusions] To apply excision and skin flap repair the ulcer uounds can improve qualify of life for chest wall recrudesce ulcer for postoperative breast cancer, and combined treatment can make their life longer.
