高渗盐水与糜蛋白酶雾化吸入诱导痰技术对痰检肺癌阳性率的比较研究

摘    要:目的 比较高渗盐水与糜蛋白酶雾化吸入诱导痰技术在痰检肺癌方面的差异。方法 选取临床拟诊肺癌病例60例,随机分为高渗盐水组和糜蛋白酶组,分别雾化吸入后留痰送检,以纤支镜检查、穿刺活检或术后病理为对照标准,比较两组肺癌检出率的差异。结果 高渗盐水组平均收集痰液量显著多于糜蛋白酶组(P<0.05),高渗盐水组留痰标本合格率显著高于糜蛋白酶组(P<0.05),两组痰检肺癌的检出率分别是77.8%和43.6%,差异有统计学意义(P<0.05)。结论 高渗盐水雾化吸入诱导痰检肺癌优于糜蛋白酶雾化吸入。

A comparative study of induced sputum collecting technique for lung cancer cell detection by sputum examination with inhalating atomized hypertonic saline and chymotrypsin solution
Abstract:Objective To compare the validity between inhalating atomized hypertonic saline and chymotrypsin solution in collecting sputum for detecting lung cancer cell. Methods Sixty cases were enrolled for lung cancer suspected with clinical data, who were randomly divided into hypertonic saline group and chymotrypsin group. They were detected lung cancer cell by collecting sputum with inhalating atomized hypertonic saline and chymotrypsin solution. The sputum examination results were proved by fiberoptic bronchoscopy examination, biopsy or postoperative pathology. Results The average amount of collected sputum is significantly more in hypertonic saline group than that in chymotrypsin group (P<0.05). The qualified rate of sputum specimens from hypertonic saline group is significantly higher than that from chymotrypsin group (P<0.05). The true positive rates were 77.8% and 43.6% in hypertonic saline group and chymotrypsin group respectively. There is statistically significant difference between them (P<0.05). Conclusion Hypertonic saline is better than chymotrypsin solution with atomization inhalation for collecting sputum in detecting lung cancer cell.
