摘    要:目的 探讨 p2 7、PCNA在子宫内膜增生过长、腺癌中的表达及意义。 方法 应用免疫组化SP法检测子宫内膜单纯和复杂性增生过长、不典型增生、腺癌中p2 7、PCNA蛋白表达。结果 p2 7蛋白高表达 (++)率在子宫内膜单纯和复杂性、不典型增生过长、腺癌中分别为 6 6 .7% (2 4 / 36 )、35 % (7/ 2 0 )、30 % (10 / 33) ,在单纯和复杂性增生过长、腺癌间差异有显著性 (P<0 .0 5 )。p2 7蛋白表达与腺癌组织分化程度、有无肌层浸润无关 (P >0 .0 5 )。PCNA表达在子宫内膜单纯和复杂性、不典型增生及腺癌中逐级递增 ,差异有显著性 (P <0 .0 1)。子宫内膜增生过长组中PCNA与 p2 7蛋白表达负相关 (P <0 .0 1) ,腺癌中p2 7与PCNA不相关。 结论 p2 7蛋白表达减少与子宫内膜腺癌发生相关 ,腺癌的发生、发展与子宫内膜细胞增殖活性提高有关。

Abstract:ObjectiveTo investigate the expression and significance of p27 and PCNA in endometrial hyperplasia and endometrial adenocarcinoma. Methodsp27 and PCNA expression in paraffin-embedded sections of 33 endometrial adenocarcinoma, 36 simple and complex endometrial hyperplasia and 20 atypical hyperplasia were detected by immunohistochemical SP assay. ResultsThe rate of the strong immunostaining of p27 in simple and complex hyperplasia, atypical hyperplasia and endometrical adenocarcinoma were 66.7%, 35% and 30%, respectively.There was a significant difference betweenendometrial adenocarcinoma and simple and complex hyperplasia (P<0.05). The expression of p27 was not correlated with histological grade or muscles invasion (P>0.05). PCNA expression increased from simple and complex hyperplasia to atypical hyperplasia and development of endometrial adenocarcinoma. An inverse correlation was found between p27 and PCNA expression in endometrial hyperplasia(P<0.01). ConclusionReduced or absent p27 expression may play an important role in the genesis of endometrial adenocarcinoma. The genesis and development of endometrial adenocarcinoma is related to the augmentation of endometrial cell proliferative activity.
