肝切除术治疗原发性肝癌：附588例临床分析

对1959年9月～1993年4月行肝癌肝切除588例进行回顾，第一阶段（1959年9月～1985年12月）施行传统肝切除术142例，均为直径＞5cm中晚期肝癌，手术死亡率11％，术后3年和5年生存率分别为18％、7％。第二阶段（1986年1月～1993年4月）行肝癌肝切除术446例，以肝段切除及癌肿局部切除为主，采用多种技术改良及高新医疗设备，并予以化疗、免疫治疗等综合措施，手术死亡率＜2％，术后1年、3年、5年生存率分别为89％、55％、26％。

Hepatic Resection for the Treatment of Primary Liver Cancer-A Clinical Analysis of 588 Cases

From September 1959 to April 1993, 588 patients with hepatocellular carcinoma underwent hepaticresection. One group of 142 cases with symptomatic and advanced tumours larger than 5 cm in diameter fromSeptember 1959 to December 1985 received regular hepatectomies. The operation mortality was 11% and 3-year,5-year survival rates were 18%,7% respectively in this group. Of the 142 cases, 5 cases underwent orthotopic liv-er transplantation and they died 42 days after operation averagely. The second group of 446 cases most of themfrom January 1986 to April 1993 had hepatic resections in form of segmentectomies and local tumour resections.The technics were improved with usage of modern advanced instruments. The operation mortality was decreased toless than 2%. In addition to hepatic resection, composite treatment such as chemotherapy and immunotherapywere administered postoperatively. One-year, 3-year and 5-year survival rates were 89%, 55 %,and 26% respec-tively. Depending upon the above results,the authors profoundly discussed the problems about the diagnosis andtreatment of small liver cancer, the recurrence and mornitor of the tumours after resection, and the two-stage re-sectioh of the tumours etc.
