检测强直性脊柱炎患者人类白细胞抗原B27的临床意义

目的探讨人类白细胞抗原B27(HLA-B27)在强直性脊柱炎(AS)诊断及鉴别诊断中的临床价值。方法用流式细胞仪检测53例AS患者、42例背腰腿疼痛患者以及30例健康体检者3组人群的HLA-B27抗原。结果 AS患者、背腰腿疼痛患者、健康体检者3组人群HLA-B27阳性率分别为90.6%、7.1%、6.7%,AS患者组HLA-B27阳性率明显高于背腰腿疼痛患者、健康体检者,差异有统计学意义(P<0.01),背腰腿疼痛组、健康体检组之间差异无统计学意义(P>0.05)。结论 FCM检测HLA-B27可为AS的诊断、鉴别诊断提供重要的依据。

Clinical value of HLA-B27 detection in patients with ankylosing spondylitis

Objective To study the clinical value of HLA-B27 detection for diagnosing and differentially diag- nosing ankylosing spondylitis（AS）. Methods HLA-B27 antigen was detected by flow cytometry（FCM） in 53 cases of AS（AS group）, 42cases of baekleg pain（backleg pain group）, and 30 healthy individuals with physical examination （control group）. Results The HLA-B27 positive rates were 90.6% in AS group,which was significantly higher than 7. 1% in the backleg pain group and 6.7% in the control group, difference showing statistical significance（P〈0.01）. There was no statistical difference between the baekleg pain group and the control group（P〉0.05）. Conclusion The detection of HLA-B27 by FCM could be an effective method for the diagnosis and differential diagnosis of AS.
