乳腺钙化对超声诊断乳腺良恶性病变的临床意义

摘    要:目的探讨乳腺钙化的声像图特征,评价乳腺钙化对乳腺良恶性病变的诊断价值。方法回顾性分析72例乳腺肿瘤患者的术前超声表现,并与穿刺活检及手术病理结果对照,分析乳腺良恶性病变的钙化类型与病理类型的关系及其诊断价值。结果本组72例中,恶性病变43例,出现钙化39例,未出现钙化的4例,检出率为90.69%(39/43);良性病变29例,出现钙化6例,未出现钙化的23例,检出率为20.68%(6/29)。由此可见,恶性病变合并钙化机率明显高于良性病变,两者差异有统计学意义(P<0.01)。72例乳腺肿块术前超声与病理诊断符合率为90.28%(65/72),误诊7例(9.72%)。结论超声能有效地检出和识别乳腺病变内微小钙化,而钙化征象对乳腺癌的诊断及鉴别诊断具有很大的价值。

Abstract:Objective To investigate the ultrasonographic features of breast calcification and evaluate the value of breast calcification in the diagnosis of benign and malignant breast lesions.Methods The preoperative ultrasonographic features of 72 cases of breast cancer were analyzed retrospectively and compared with the results of biopsy and operativepathological findings.The correlation between the types of calcification and pathological types of breast benign and malignant lesions and the value of diagnosis were analyzed.Results Of the 72 cases,there were 43 cases of malignant lesions,39 cases calcified,no calcification in 4 cases,a detection rate of 90.69%(39/43); Benign lesions in 29 cases,calcification in 6 cases,no calcification in 23 cases,and the detection rate was 20.68%(6/29).Thus,the rate of malignant lesions associated with calcification was significantly higher than that of benign lesions,and the difference was statistically significant(P<0.01).The coincidence rate between preoperative ultrasonography and pathological diagnosis was 90.28%(65/72) in 72 cases of breast masses,and 7 cases were misdiagnosed,9.72%(7/72).Conclusion Ultrasound can effectively detect and identify tiny calcifications in breast lesions,and calcification signs are of great value in the diagnosis and differential diagnosis of breast cancer.
