小剂量米氮平预防卒中后抑郁的疗效观察

【】 目的 观察小剂量米氮平对脑卒中后抑郁(post-stroke depression, PSD)的预防作用及对卒中后患者神经功能恢复的影响。方法 将102例首次卒中患者随机分为对照组和治疗组,两组患者均根据自身病情给予对症治疗,治疗组在此基础上加用15 mg米氮平片睡前口服,对照组给予同等剂量安慰剂。分别于治疗前、治疗后4周、治疗后8周对两组患者进行汉密尔顿抑郁量表(Hamilton depression scale, HAMD)评分和改良巴氏指数(Modified Barthel Index,MBI)评分,评估小剂量米氮平对PSD的预防及疗效。结果 治疗4周及8周后,治疗组PSD发生率均低于对照组,HAMD评分低于对照组,MBI评分高于对照组,差异具有统计学意义(P < 0.05)。结论 对于脑卒中患者预防性给予小剂量米氮平能减少PSD的发生,并能促进卒中患者神经功能的恢复。

Clinical observation of low-dose mirtazapine for preventing post-stroke depression

Objective To observe the clinical effects of low-dose mirtazapine on preventing post-stroke depression and the recovery of neurological functions in first-ever stroke patients. Methods A total of 102 patients with first-ever stroke were randomly divided into control group and treatment group. In addition to symptomatic treatment, patients in the treatment group were treated with mirtazapine 15 mg per night, patients in the control group were treated with equal dose of placebo. The Hamilton depression scale (HAMD) scores and Modified Barthel Index (MBI) scores were evaluated before treatment, 4 weeks and 8 weeks after treatment. Results Compared with the control group, the incidence of PSD and the HAMD scores in treatment group were obviously reduced, and the MBI scores were significantly increased (P < 0.05). Conclusions Prophylactic administration low-dose mirtazapine to stroke patients can reduce the incidence of PSD and promote the recovery of neurological functions after stroke.
