妊娠期糖尿病患者甲状腺功能状态的研究

摘    要:目的了解绵阳地区妊娠期糖尿病患者中甲状腺功能指标的分布情况并分析其临床意义。方法回顾性分析2016年1月至2016年12月我院内分泌科及产科就诊的148例妊娠期糖尿病患者(研究组)及同期118例糖耐量正常孕妇(对照组)的促甲状腺激素(Thyroid-stimulating hormone,TSH)、游离三碘甲状腺原氨酸(Free triiodothyronine,FT3)、游离甲状腺激素(Free thyroxin,FT4)。结果研究组甲状腺功能异常的患病率高于对照组,研究组甲状腺功能减退症(甲减)、低T4血症的患病率高于对照组(P<0.05);研究组FT3、FT4水平低于对照组(P<0.05);不同年龄段患者TSH、FT3、FT4水平无统计学差异(P>0.05);不同胎次患者TSH、FT3、FT4水平无统计学差异(P>0.05)。结论妊娠期糖尿病患者更易出现甲状腺激素的异常,临床应加大筛查力度。高龄、二胎并不增加孕妇甲状腺疾病患病风险。

Abstract:Objective To investigate the distribution of thyroid function indexesin gestational diabetes mellitus patients in Mianyang and analyze its clinical significance.Methods A retrospective analysis of the thyroid stimulating hormone(Thyroid-stimulating,hormone,TSH),free triiodothyronine three(Freetriiodothyronine,FT3),free thyroid hormone(Free thyroxin,FT4) of 148 cases of gestational diabetes patients between January 2016 and December 2016 in department of Endocrinology and obstetrics of our hospital(Study Group) and 118 cases of pregnant women with normal amount of glucose tolerance during the same period(control group).Results The incidence of thyroid dysfunction in the study group was higher than that in the control group,and the incidence of hypothyroidism and low T4 in the study group was higher than that in the control group(P<0.05); The levels of FT3 and FT4 in the study group were lower than those in the control group(P<0.05); There was no significant difference in TSH,FT3,FT4 levels between different patients with age groups(P> 0.05); And there was no significant difference in TSH,FT3,FT4 levels between patients with different parities(P> 0.05).Conclusions Patients with gestational diabetes mellitus are more likely to have abnormal thyroid hormone,and clinical screening should be intensified.Advanced age and second child do not increase the risk of thyroid disease in pregnant women.
