磷霉素对肠杆菌科细菌的抗菌作用研究

摘    要:目的评价磷霉素对肠杆菌科细菌的抗菌作用。方法收集3家医院2012年1月到2014年6月非重复分离的150株肠杆菌科细菌,包括大肠埃希菌、肺炎克雷伯菌和粘质沙雷菌各50株,其中大肠埃希菌、肺炎克雷伯菌和粘质沙雷菌各包括25株碳青霉烯敏感株和25株碳青霉烯耐药株,用琼脂稀释法测定磷霉素药敏。结果150株肠杆菌科细菌对磷霉素的耐药率为25.3%,大肠埃希菌、肺炎克雷伯菌、粘质沙雷菌对磷霉素的耐药率分别为2.0%、48.0%、26.0%;大肠埃希菌中碳青霉烯敏感株与碳青霉烯耐药株磷霉素耐药率分别为4.0%和0%,肺炎克雷伯菌中碳青霉烯敏感株与碳青霉烯耐药株磷霉素耐药率分别为0%和56.0%,粘质沙雷菌中碳青霉烯敏感株与碳青霉烯耐药株磷霉素耐药率分别为20.0%和24.0%。结论磷霉素对肠杆菌科细菌体外有较好的抗菌活性。

Abstract:Objective To investigate the antimicrobial susceptibility of Enterobacteriaceae to fosfomycin. Methods A total of 150 non-repetitive Enterobacteriaceae isolates were collected from the Second Affiliated Hospital of Zhejiang University School of Medicine, Hangzhou Traditional Chinese Medicine Hospital and Zhejiang Provincial People''s Hospital between January 2012 and June 2014, including 50 strains of Escherichia coli, 50 strains of Klebsiella pneumoniae and 50 strains of Serratia marcescens. Among all these isolates, there were 25 carbapenem-sensitive strains and 25 carbapenem-resistant strains for each bacteria, respectively. Susceptibility of fosfomycin was determined by agar dilution method. Results Overall 25.3% of 150 Enterobacteriaceae isolates were resistant to fosfomycin. The resistance rate of Escherichia coli, Klebsiella pneumoniae and Serratia marcescens was 2.0% , 48.0% and 26.0% , and the resistance rate of carbapenem-sensitive strains vs. carbapenem-resistant strains was 4.0% vs. 0%, 0% vs. 56.0% and 20.0% vs. 24.0% for Escherichia coli, Klebsiella pneumoniae and Serratia marcescens, respectively. Conclusion Fosfomycin demonstrates certain activity against Enterobacteriaceae in vitro.
