| Abstract: | Pediatric nurses can assist adolescents with chronic conditions to make the necessary linkages to adult services systems, enabling them to access needed services. Knowledge of the adult services available for adolescents and young adults will enable them to realize their goals for adulthood. This article provides an overview of services available to assist high school students with chronic conditions in making the transition to postsecondary settings. |
