超声乳化白内障吸除术晶状体后囊膜破裂患者的并发症和远期预后

目的　探讨超声乳化白内障吸除术中晶状体后囊膜破裂患者并发症的发生情况和远期预后。方法　对 6 6例 (6 6只眼 )超声乳化白内障吸除术中晶状体后囊膜破裂患者进行回顾性总结 ,记录患者晶状体后囊膜破裂或 (和 )悬韧带断裂的发现时间、玻璃体脱出情况和清除方法、人工晶状体的植入情况、手术前和后的最佳矫正视力、术后并发症情况。平均随访时间 (11± 8)个月。结果　在 6 6例患者中 ,术后最佳矫正视力≥ 0 5者 5 2例 (5 2只眼 )。术后并发症包括角膜水肿 (42只眼 )、瞳孔变形 (5只眼 )、眼压升高 (4只眼 )、黄斑囊样水肿 (3只眼 )、晶状体皮质残留 (2只眼 )、视网膜脱离 (2只眼 )、虹膜缺损 (1只眼 )和人工晶状体脱位 (1只眼 )。结论　超声乳化白内障吸除术中晶状体后囊膜破裂 ,可引发一系列手术并发症 ;只要通过正确处理 ,患者术后仍可获得较好的视力恢复

Visual outcome and complications after posterior capsule rupture during phacoemulsification

Objective To determine the visual outcome and incidence of complications after posterior capsule rupture during phacoemulsification. Methods Sixty six eyes of 66 patients which sustained posterior capsule rupture were retrospectively studied. Result Fifty two out of 66 eyes had best corrected visual acuity of 20/40 or better. The complications included corneal edema (42 eyes), iris injury (1 eye), pupil decentration (5 eyes), intraocular hypertension (4 eyes), intraocular lens dislocation (1 eye), cystoid macular edema (3 eyes), remain of lens material (2 eyes) and retinal detachment (2 eyes). Conclusions Phacoemulsification, even if complicated by posterior capsule rupture, is compatible with good visual outcome. Immediate proper management of complications is important.
