脑脊液置换术治疗蛛网膜下腔出血疗效观察

目的探讨腰穿脑脊液置换术对蛛网膜下腔出血（SAH）疗效及临床应用价值。方法以51例SAH患者为研究对象，随机分为治疗组（26例）和对照组（25例）。对照组为常规内科治疗，治疗组在常规治疗基础上应用腰穿脑脊液置换术。结果治疗组头痛持续时间、脑膜刺激征症状消失时间及脑脊液恢复正常时间较对照组缩短，差异具有统计学意义（P〈0．01）；急性脑积水、脑血管痉挛、死亡率明显降低，经统计学处理差异有显著性（P〈0．05）。结论腰穿脑脊液置换术治疗蛛网膜下腔出血安全有效，可减少并发症及死亡率。

Cerebrospinal Fluid Replacement by Lumbar Puncture for Subarachnoid Hemorrhage:Clinical Effect

Objective To explore the curative effect and the value of clinical application of cerebrospinal fluid replacement by lumbar puncture in subarachnoid hemorrhage （SAH）. Methods Fifty-one cases of SAH were randomLy divided into two groups： treatment group with 26 cases in it and control group with 25 cases in it. Both groups were given conventional treatment and the cerebrospinal fluid by lumbar puncture was added to the treatment group. Results Duration of headache and disappeared time of meningeal stimulation symptom and time of CSF returning to normal in the treatment group were less than those in the control group, with a significant difference between the two groups （P〈0.01）. The rates of hydrocephalus, cerebral vasospasm（cvs）and death were decreased obviously,with a significant difference（P〈0.05 ）. Conclusion The cerebrospinal fluid replacement for subarachnoid hemorrhage is safe and effective, and can reduce the incidence of complications and mortality.
