摘    要:目的观察用不同方法注射O3治疗腰椎间盘突出症对O3在椎间盘内分布的影响,并探讨其与疗效的关系。方法将200例腰椎间盘突出症(LDH)患者随机分成2组,观察组100例接受弯套针旋转多向注射法注射O3治疗,对照组100例采用常规方法注射治疗。注射结束后立即在CT下观察椎间盘内O3分布情况,并于6个月后统计疗效。选取50例患者于治疗6个月后复查CT,测量改良椎间盘突出指数(AIDH)分析突出物大小的变化。结果观察组痊愈显效率为85.00%;对照组为69.00%(χ2=7.23,P〈0.01)。观察组突出物内有O3和O3在盘内分散分布的总例数明显多于对照组(P〈0.001、0.01)。O3呈分散分布者及突出物内有O3者疗效更好(P〈0.001)。治疗6个月后椎间盘内O3分散分布者突出物减少程度明显大于聚集分布者(P〈0.05)。结论弯套针旋转多向注射法可提高O3在椎间盘内分散分布及在突出物处分布的比率,使突出物明显缩小或消失,治疗LDH的疗效明显优于常规注射方法 。

Abstract:Objective To observe the influence of different ozone injection methods in the treatment of lumbar disc herniation(LDH)on ozone distribution in the intervertebral discs,and to discuss the relation of ozone distribution with the curative effect.Methods Totally 200 patients of LDH were divided into two groups randomly:100 patients in observation group were injected ozone with curving sheath-needle multi-direction rotating injection(CSNMRI);100 patients in the control group were treated with conventional injection method.Ozone distribution in intervertebral discs was viewed immediately under CT when the injection completed.The curative effect of both groups was evaluated after 6 months.Fifty patients were chosen to recheck CT at 6 months after treatment.The changes of protrution size were analyzed by measuring amendatory index of disc herniation(AIDH).Results The cured and markedly effective rate was significantly different between observation group(85.00%)and control group(69.00%)(χ2=7.23,P0.01).The total number of patients of ozone within the protrusion or dispersively distributed in the disc was significantly more in observation group than that in control group(P0.001,0.01).The curative effect was better when ozone was dispersive in the disc or lay inside the protrusion(P0.001).Six months after treatment,the protrusion size decreased more when ozone was dispersive in the disc than when it was collective(P0.05).Conclusion CSNMRI can improve the distribution rate of ozone dispersal in the intervertebral discs and within the protrusion,sequentially the protrusion deflation or disappearance is more remarkable.CSNMRI has better curative effect than conventional method.
