卵巢上皮性肿瘤中微血管密度、微淋巴管密度与临床病理因素及预后的关系

摘    要:目的:计数卵巢上皮性肿瘤中微血管密度及微淋巴管密度,探讨其与临床病理因素间的关系,及对患者预后的影响。方法:采用免疫组织化学方法检测10例正常卵巢上皮,48例良性卵巢上皮性肿瘤,47例交界性卵巢上皮性肿瘤以及205例恶性卵巢上皮性肿瘤组织中CD34及D2-40的表达。结果:正常卵巢组织中的MVD、LMVD值较良性、交界性及恶性卵巢上皮性肿瘤组织中的值显著降低,差异均有统计学意义(P均<0.05)。恶性上皮性卵巢肿瘤组织中MVD、LMVD值与肿瘤的临床分期、分化程度、有无淋巴结转移有关,差异有统计学意义(P均<0.05),但与患者的年龄无关(P>0.05)。多血管组及多淋巴管组的患者术后平均生存时间较少血管组及少淋巴管组显著降低,差异有统计学意义(P均<0.05),两者的表达水平均与患者术后生存时间呈负相关(P<0.05)。结论:随着卵巢上皮性肿瘤恶性程度的增加,MVD及LMVD值升高,与肿瘤的低分化、高分期、淋巴结转移有关,预示患者预后不良。

Relationship between microvessel density,lymphatic microvessel density and clinicopathological factor,prognosis of patients with epithelial ovarian tumor

Abstract:Objective:To count the microvessel density(MVD) and lymphatic microvessel density(LMVD) in epithelial ovarian tumors,explore their relationships with clinicopathological factor and the effects on the prognosis.Methods:Immunohistochemical method was used to detect the expression levels of CD34 and D2-40 in 10 cases with normal ovarian epithelium,48 cases with benign epithelial ovarian tumor,47 cases with boundary epithelial ovarian tumor,and 205 cases with malignant epithelial ovarian tumor.Results:MVD and LMVD in normal ovarian tissue were significantly lower than those in benign epithelial ovarian tumor tissue,boundary epithelial ovarian tumor tissue,and malignant epithelial ovarian tumor tissue(P<0.05).In malignant epithelial ovarian tumor tissue,MVD and LMVD were related to clinical stages,degrees of differentiation and lymph node metastasis or not(P<0.05);but they were not correlated with the age of the patients(P>0.05).The average postoperative survival time in multiple vessels and lymph vessels group were significantly shorter than that in less vessels and lymph vessels group(P<0.05),there was a negative correlation between the expression levels of CD34 and D2-40 and the postoperative survival time(P<0.05).Conclusion:MVD and LMVD increase with the increase of malignant degree of epithelial ovarian tumor,which are related to poor differentiation,high clinical stage,and lymph node metastasis,and they indicate poor prognosis.
