子宫内膜癌455例临床病例分析

目的:探讨两种类型子宫内膜癌的发病年龄、手术-病理分期、组织学分级、肌层浸润及雌孕激素受体表达的差异,以期增强对非子宫内膜样癌的认识及重视。方法:对455例子宫内膜癌患者的临床病理资料进行回顾性分析。结果:两种类型子宫内膜癌的平均发病年龄、在绝经前后患者中的构成比、组织学分级、手术-病理分期、肌层浸润深度及雌、孕激素受体表达及淋巴结转移情况有统计学差异。结论:非子宫内膜样癌与子宫内膜样腺癌相比,患者发病年龄较大;在已绝经患者中所占比例远高于子宫内膜样腺癌;Ⅲ/Ⅳ期、中、低分化、雌孕激素受体阴性的比例明显高于内膜样腺癌,更易发生深肌层浸润和淋巴结转移。

Clinical analysis on 455 cases with endometrial cancer

Objective:To explore the differences of onset age,surgical-pathological stages,histological grades,myometrial invasion,estrogen receptor expression,and progestogen receptor expression between two types of endometrial cancer,in order to enhance the cognition and attention to non-endometrioid adenocarcinoma. Methods:The clinicopathological data of 455 cases with endometrial cancer were analyzed retrospectively. Results:There was significant difference in the average onset age,the proportions of postmenopausal patients and premenopausal patients,histological grades,surgical-pathological stages,the depth of myometrial invasion,estrogen receptor expression,progestogen receptor expression,and lymph node metastasis between the patients with two types of endometrial cancer. Conclusion:Compared with the patients with endometrioid adenocarcinoma,the onset age of the patients with non-endometrioid adenocarcinoma is older;the proportion of postmenopausal patients among the patients with non-endometrioid adenocarcinoma is significantly higher than that among the patients with endometrioid adenocarcinoma;the proportions of stage Ⅲ/Ⅳ,moderate and poor differentiation,negative estrogen receptor and progestogen receptor among the patients with non-endometrioid adenocarcinoma are significantly higher than those among the patients with endometrioid adenocarcinoma,the patients with non-endometrioid adenocarcinoma are prone to deep myometrial invasion and lymph node metastasis.
