术中超声在区域型肝内胆管结石精准肝切除中的应用

目的 探讨术中超声在治疗区域型肝内胆管结石采用精准肝切除的应用价值。方法 回顾性分析87例区域型肝内胆管结石患者的临床资料,在术中超声引导下行精准肝切除,术后随访,评价术中情况、术后恢复及远期效果。结果 87例患者均手术成功,术后结石残留8例（9.2%）,患者术后均顺利康复,术中及围手术期无严重并发症发生,无死亡病例。术后规律随访4年,结石复发15例（17.2%）。结论 术中超声引导下行精准肝切除对区域型肝内胆管结石患者有很高的临床应用价值。

The Application of Intraoperative Ultrasound in the Precise Liver Resection to Therapy Regional Hepatolithiasis

Objective To discuss the application of the intraoperative ultrasound（IOUS）in the precise liver resection to therapy regional hepatolithiasis.Methods The clinical datum of 87 cases of patients with the regional hepatolithiasis who undergone precise liver resection guided by the intraoperative ultrasound were retrospectivel y analyzed,including the regular follow-up after operation,the evaluations about the effect of intraoperative,postoperative recovery and long-term conditions.Results All of the 87 patients had the successful operations,in which the 8cases had residual stones after operation（9.2%）and 79 cases had recovered well without serious complications and death both in operation and perioperative period.Then 15 cases were found to have a cholecystolithiasis recurrence（17.2%）during the 4-year regular follow-up.Conclusion The precise liver resection guided by the IOUS has a very high clinical value in the treatmenl for patients with the therapy regional hepatolithiasis.
