p16、bcl-2基因蛋白在胃癌和癌前病变的表达及意义

摘    要:目的探讨p16、bcl-2基因在胃癌发生、发展中的重要作用以及在胃癌早期诊断中的意义.方法运用免疫组化技术检测胃癌及癌前病变中p16、bcl-2基因蛋白的表达.结果p16基因蛋白阳性表达率在轻、中、重度异型增生分别为66.67%、50.00%、37.50%,在胃癌为28.57%;bcl-2基因蛋白阳性表达率在轻、中、重度异型增生分别为28.57%、33.33%、37.50%,在胃癌为68.57%.p16基因蛋白的表达在轻度异型增生组及胃癌组有显著差异(P<0.001);bcl-2基因蛋白的表达在轻度异型增生组与胃癌组(P<0.025)、中度异型增生组与胃癌组(P<0.01)均有显著差异.p16与bcl-2基因蛋白表达呈显著性负相关.结论p16基因的失活与bcl-2基因的过表达在胃癌的发生、发展中起重要作用;重度异型增生是一种重要的癌前病变;对胃异型增生粘膜进行p16、bcl-2基因检测有助于胃癌的早期诊断和治疗.

Abstract:Objective To investigate the important role in the progress of carcinogenesis of stomach and the significance of p 16 and bcl-2 gene in the diagnosis of early gastric cancer. Methods Applying standard immunohistochemical S-P techniques to detecte p 16 and bcl-2 protein expression in dysplasia gastric mucosa and gastric carcinoma. Results The positive rate of p 16 in mild, moderate and severe dysplasia gastric mucosa were 66.67%, 50.00% and 37.50%; in gastric cancer it was 28.57%.The positive rate of bcl-2 in mild, moderate and severe dysplasia gastric mucosa were 33.33%, 37.50% and 62.5%; in gastric cancer it was 68.57%.The positive rates of p 16 had significant difference between mild dysplasia and gastric cancer (P <0.001).The positive rates of bcl-2 had significant difference between mild dysplasia and gastric cancer (P<0.025), moderate dysplasia and gastric cancer(P <0.05),and there was significant negative relation between the positive expression of p 16 and bcl-2 gene protein. Conclusions The inactivation of p 16 gene and over expressed of bcl-2 gene play an important role in the progress of carcinogenesis of stomach.The severe gastric dysplasia is the important precancerous lesion.Detecting the p 16 and bcl-2 gene in dysplasia gastric mucosa could predict the canceration trend and contribute to the diagnosis of gastric cancer early.
