儿童恙虫病并心脏损害的临床观察

目的观察儿童恙虫病并发心脏损害的临床特点。方法对43例合并心脏损害的患儿的临床症状与体征进行观察，并进行心电图、心肌酶谱和超声心动图检查。结果该组患儿临床表现有心音低钝、心律不齐、心动过缓及心动过速等，同时出现心电图异常改变和心肌酶谱活性持续升高，当病情好转时两者趋向正常。结论儿童恙虫病可伴有不同程度的心脏损害，治疗中应注意保护心脏功能。心电图和心肌酶谱的改变可作为预测心脏受损的重要指标。

Clinical study of children with scrub typhus complicated by heart damage

Objective To survey the clinical character of children with the scrub typhus complicated by heart damage. Methods To observe the clinical symptoms and objective signs of 43 children with complicated heart damage, and examine cardiac muscle zymogram, electrocardiogram and echocardiography. Results The clinical manifestations of the cases included low-pitched heart sound arrhythmia, bradycardia and tachycardia, etc, mean- while there were electrocardiographic abnormality and increased activity of persistence in cardiac muscle zymogram, both all trended to normal when the pathogenetic condition took a favorable turn. Conclusion The scrub typhus in children may accompany different extent heart damage and more attention should be paytd to protect the cardiac function during the treatment. The alteration of electrocardiogram and cardiac muscle zymogram can be the important indexes which may predict the heart damage.
