超声引导经皮肾镜应用第三代超声/弹道碎石机治疗上尿路结石

目的:探讨超声引导经皮肾镜应用第三代超声/弹道碎石清石系统治疗上尿路结石的临床疗效。方法:回顾性分析我院自2006年10月～12月经皮肾镜应用第三代超声/弹道碎石清石系统治疗上尿路结石39例的临床资料(超声/弹道碎石组),并以我院自2006年1月～9月微创经皮肾穿刺气压弹道碎石术治疗上尿路结石126例的临床资料作为对照组(气压弹道碎石组),比较两组患者的平均手术时间、平均结石处理时间、一期结石清除率及平均手术失血量的差异。结果:超声/弹道碎石组的平均手术时间、平均结石处理时间均较对照组短(P<0.05);超声/弹道碎石组的一期结石清除率高于对照组(P<0.05);超声/弹道碎石组的平均手术失血量较对照组少(P<0.01)。结论:超声引导经皮肾镜超声/弹道碎石取石术是治疗上尿路结石的安全、高效的方法,临床效果显著。

Treatment of Upper Urinary Tract calculi with Percutaneous Nephrolithotomy Using Third Generation Swiss LithoClast Master under Ultrasound Guidance

Objective To evaluate the clinical efficacy of percutaneous nephrolithotomy using third generation Swiss LithoClast Master under ultrasound guidance in treating upper urinary tract calculi.Methods We retrospectively analyzed 39 patients from Oct 2006 to Dec 2006 in our hospital who underwent percutaneous nephrolithotomy using third generation Swiss LithoClast Master under ultrasound guidance for upper urinary tract calculi(Swiss LithoClast Master treatment group),as a contrast,we also investigated 126 patients from Jan 2006 to Sep 2006 in our hospital who performed by minimally invasive percutaneous nephrolithotomy with pneumatic lithotripsy for upper urinary tract calculi(pneumatic lithotripsy contrast group).Clinical data including the average time for whole surgery,the average time for stone management,the phase I stone-free rate and the average blood loss volume were made comparisons respectively in the two groups.Results Compared with the pneumatic lithotripsy contrast group,average times for whole surgery and for stone management were remarkably decreased in Swiss Litho Clast Master treatment group(P<0.05),the phase I stone-free rate was significantly increased(P<0.05) and the average blood loss volume was markedly reduced in Swiss LithoClast Master treatment group(P<0.01).Conclusion The treatment of upper urinary tract calculi by percutaneous nephrolithotomy using third generation Swiss LithoClast Master appears to be safe and efficacious under ultrasound guidance.
