青海地区丙型肝炎基因亚型与肝损伤、糖脂代谢

摘    要:目的探讨青海地区丙型肝炎基因亚型与肝损伤、糖脂代谢相关性。方法收集149例抗-HCV、HCV-RNA均阳性的慢性丙肝患者血清,采用HCV-RNA荧光定量PCR法和反向免疫杂交法检测HCV-RNA载量和基因型,分析青海地区丙型肝炎基因亚型与肝损伤、糖脂代谢相关性。结果 149例检出1 b型61例、2 a型52例、3 b型18例、3 a型6例、6 a型0例、1 b/2 a型1例;性别以男性为主;年龄主要分布在30-60岁。1 b、2 a、3 b型丙肝患者CHOL存在统计学差异(P〈0.05);1 b、3 b型丙肝患者GLU含量高,和2 a型患者比较存在统计学差异(P〈0.05);其他指标无统计学差异。结论青海地区人群中HCV的基因亚型以1 b和2 a型为主,混有少量3 b和3 a型。1 b、2 a和3 b型糖、脂代谢存在差异性,HCV感染后易合并糖尿病和高脂血症。

Abstract:Objective To investigate the genetic subtype of hepatitis C( HCV) and its relationship with liver damage and glycolipids metabolism at Qinghai region. Methods Both anti- HCV and serum HCV- RNA positive149 cases of patients with chronic hepatitis C were selected as subject. HCV- RNA load and genotype were detected by Fluorescence Quantitative PCR and Reverse Immune Hybridization Assay. The genetic subtypes of hepatitis C and its relationship with liver damage and glycolipids metabolism at Qinghai area was analyzed. Results Among 149 cases of patients with hepatitis C,61 cases of type lb,52 cases of 2a type,18 cases of 3 b,6 cases of 3 a,0 cases of6 a,1 cases of 1 b /2 a were detected; Gender was men- dominated; ages were mainly in the 30 to 60 years old.There was significant difference( P〈0. 05) in CHOL of 1 b,2 a and 3 b hepatitis C patients; patients with type 1 b and 3 b showed high levels of GLU,and compared them with type 2 a patients showed statistical difference( P〈 0.05). Conclusions The genetic subtypes of HCV in Qinghai population are mainly dominated by type lb and2 a,mixed with a small amount of type 3 b and 3 a. The metabolism of glucose and lipid in type 1 b,2 a and 3 b are presence of difference. HCV infection is prone to complicate with diabetes and hyperlipidemia.
