中心性浆液性脉络膜视网膜病变的频域光相干断层扫描观察

摘    要:目的:观察中心性浆液性脉络膜视网膜病变(CSC)在频域光相干断层扫描(FD-OCT)中的图像特征。方法:对确诊为CSC患者29例34只眼行频域光相干断层扫描(FD-OCT)检查,采用二极管反射的红外光对眼底黄斑部进行线性水平扫描,扫描面积6mm×6mm,扫描深度2mm,轴向分辨率5μm,横向分辨率18μm,扫描模式512×128。从断层成像图和冠状面成像图观察CSC的FD-OCT图像特征。结果:所有患眼中出现神经上皮层脱离32只眼(94.1%),其中,神经上皮脱离区内合并色素上皮脱离(PED)7只眼(20.1%);神经上皮层脱离区下视网膜色素上皮(RPE)反射信号不连续甚至消失15只眼(44.1%);脱离区内可见颗粒状高反射信号12只眼(35.3%)。病灶处IS/OS层反射信号中断消失23只眼(67.7%)。视网膜PED9只眼(26.5%);RPE呈结节状突起改变9只眼(26.5%),呈波纹状改变3只眼(8.8%)。视网膜神经上皮平均脱离高度(237.16±107.33)μm;RPE平均脱离高度(69.25±36.12)μm。结论:FD-OCT能清楚地显示CSC的形态特征并准确地发现各种细微改变,可全面、多方位地观察CSC的病理形态学变化。

Abstract:Objective:To investigate the morphological changes of central serous chorioretinopathy (CSC)by Fourier-domain optical coherence tomography(FD-OCT).Methods:FD-OCT scan was performed on 34eyes of 29 patients with the established CSC diagnosis.OCT scan of 6mm horizontal line was performed utilizing the infrared light emitted by diode,the scan depth was 2mm with a 6mm×6mm transverse area.The vertical and horizontal resolution was 5μm and 18μm respectively.The scan mode was 512×128.The morphological changes were evaluated with B-scan and C-scan.Results:The uplift of retinal neuroepithelium was found in 32 eyes(94.1%), including 7 eyes(20.1%)of pigment epithelium detachment(PED),15eyes(44.1%)of inconsecutive retinal pigment epithelium(RPE),and 12 eyes(35.3%) with bright reflect spots.The IS/OS reflects of 23 eyes(67.7%)were defected.In the abnormal changes of RPE,there were 9 eyes(26.5%)of PED,9 eyes(26.5%)of small protrusions of RPE,and 3eyes(8.8%)of wave-like RPE layer.The height of detachment of neuroepithelium and detachment of RPE was (237.16 107.33)μm and(69.25±36.12)μm respectively.Conclusion:Fourier-domain OCT can detect detailed morphologic changes in the retina of CSC patients,and enable the comprehersive observation of pathological morphology and location.
