腘动脉陷迫综合征4例报告

目的:探讨腘动脉陷迫综合征(PAES)的诊断和外科治疗经验。方法:对4例(6条下肢)PAES患者施行了外科手术治疗。所有患者的数据均收自于原始的病例资料。结果:4例患者平均年龄24.2岁(15-32岁),均为男性。2例累计双下肢。间歇性跛行是最主要的症状(6条下肢)。其中1例为Delaney’sⅡ型,另外3例为Delaney’sⅢ型。所有6条患肢均施行了腘动脉减压术,其中1条因动脉闭塞同时施行了大隐静脉旁路移植术。随访期平均10个月(30d-2年),所有患者间歇性跛行症状治愈。结论:PAES虽然罕见,但却是青年周围血管缺血患者的一个重要病因。为了取得较好的临床疗效、减少截肢率,早期诊断和外科干预是十分重要的。

Diagnosis and Surgical Treatment of Popliteal Artery Entrapment Syndrome:Report of Four Cases

Objective:To review the surgical treatment of six limbs of four patients with popliteal artery entrapment syndrome (PAES). Methods: From year 2004 to 2007, four patients (six limbs) with underwent surgery PAES in our hospital, and the clinical data were collected and analyzed. Results: There were four male patients with a mean age of 24.2 years (15-32 years), and two patients were found to have bilateral involvement. Intermittent claudication was the most frequent presenting symptom. One patient had Delaney’s type Ⅱ PAES and the other three patients had Delaney’s type Ⅲ PAES. Popliteal artery release surgery was performed in all six limbs, and this procedure was combined with a reversed long saphenous vein bypass graft in one limb because the popliteal artery was occluded. After a median follow-up of 10 months (30 days to 2 years), all patients were cured of their symptoms. Conclusion: Popliteal artery entrapment syndrome is a rare but important cause of peripheral vascular insufficiency especially in young patients. Early diagnosis and surgical intervention is imperative for good operative outcome and to prevent limb loss.
